Adults (18+) with mental health, developmental, or cognitive support needs who qualify for DBHDS/DMAS waiver services or private-pay arrangements. Each applicant is assessed individually to ensure the home is a good fit.

3. How do I apply for placement or refer someone?

You may contact us directly by phone or email. A brief intake assessment will be completed, followed by a tour and a review of the individual’s needs and support requirements.

4. Is the home licensed?

Yes. T&D Health Services, LLC complies with all Virginia DBHDS and HCBS regulations, including staffing, documentation, safety inspections, and person-centered practices.

5. What is included in the monthly cost of care?

Monthly care includes:

  • 24/7 staff supervision

  • Medication administration

  • Meals and snacks

  • Daily living support (bathing, dressing, hygiene)

  • Transportation to appointments

  • Care coordination and progress documentation

Yes. Residents receive nutritious meals daily. Special diets—diabetic, low-sodium, gluten-free, cultural or religious preferences—are fully accommodated.
